  • The main function of this position is to assist and support the individual in maximizing their "everyday life."
  • The principles and practices of Positive Approaches and Self-Determination will be the driving influences in the provision of support services.
  • The person in this position will have the responsibility of assisting each individual in the development of their Annual Support Plan, based on the results of specific assessments designed to develop outcomes appropriate to the individual's "personal vision."
  • The person in this position will maintain regular communication and contact with each individual in order to insure that all aspects of needed / necessary supports are intact.
  • The individual [or appropriate designee] will drive the focus and direction of services and supports.

  • The minimum requirement for this position will be an appropriate Bachelor's Degree from a college or university, which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, special education, or other related social sciences.
  • Two years of experience as a Social Service Aide 3 and 2 years of college level work, including 12 college credits in the areas outlined above.
  • Any equivalent combination of experience and training which includes 12 college credits, as outlined above, and 1 year of experience as a County Social Service Aide 3
  • Must also have a Valid Driver's License, State Police, FBI and Child Abuse Clearances.
  • Flexibility of time and work hours is necessary in this position. There may be a need for evening and/ or weekend hours, dependant on the needs of the individuals receiving support.
  • Support Coordinator will be responsible for Case load in Lackawanna and Susquehanna Counties, in Pennsylvania
